For Parents/Guardians of Students enrolled in Penn Manor’s Virtual School (PMVS)

Please contact your student’s home building school nurse via phone or email to schedule a time to complete these mandatory screenings.  The screening process typically takes no more than 10 minutes to complete.  

Parents who do not want their child screened at school must have the appropriate screenings completed by their private provider at the parent’s expense.  The medical provider will need to complete the grade appropriate screenings and return the signed document to the school nurse  Completed screenings may be faxed to the nurse fax number:  1-888-510-8011.

Grades K-3 – Vision, Hearing, Height & Weight
Grades 4-5 – Vision, and Height & Weight
Grade 6 – Vision, Height & Weight, and Scoliosis
Grade 7 – Vision, Hearing, Height & Weight, and Scoliosis
Grades 8-10 – Vision and Height & Weight
Grade 11 – Vision, Hearing, and Height & Weight
Grade 12 – Vision, and Height & Weight
