Reminder:

All incoming 7th graders MUST have Tdap and MCV immunizations prior to the first day of school.

All kindergarten, first, sixth and eleventh graders are required to have a physical examination.

All kindergarten, first, and fifth graders are required to have a dental examination.

Completed exams should be returned by the first day of school.

Forms may be found on the Penn Manor Nurse blog under forms tab.  Completed forms may be dropped off, mailed or faxed to your child’s school.

Sealant Saturday-February 1st

Free Oral Exams and Sealants 

Children in 1st, 2nd, and 7th grades-especially those who do not have a dentist and are unisured-can receive free oral exams and sealants.

Thanks to everyone that returned completed physical and dental forms. 

Just a reminder:

Kindergarten, 6th, and 11th grade physical examination forms are due. 

Kindergarten, 1st and 5th grade dental examinations are also due. 

All 7th graders are required to have the following immunizations upon entry into 7th grade:  TDaP and MCV

Please be sure that a copy of these immunizations have been sent to the school nurse.  If your 7th grader has not had these immunizations please contact your school nurse for further information.

If you do not have insurance that covers the physical or dental exam, please contact your school nurse, who may be able to assist you.

Physicals and Dental examination requirements for 2012/13 School Year

According to Pennsylvania School Health laws, if your child is entering kindergarten or grade 6 next year, he or she will need to have a physical exam completed.   The physical exam time is also a perfect time for a discussion with your child’s doctor about any additional immunizations that are recommended for your child.  Exams should be returned by the first day of school next year.

 

Reminder:  Students entering 7th grade are required to have the meningitis (MCV) and Tdap booster prior to the first day of school in August.

 

Dental exam for students entering grades 1 and 5 are also required.  Forms for students entering grades 1 and 5 next year will be arriving home soon in your child’s report card envelope or communication folder.
