Forms for the administration of medication while at school, can be found under the “Forms” tab on this blog site.
MEDICATION POLICY
All medications, including prescription and over-the-counter medications and essential oils, require a signed order from a licensed prescriber and a parent signature. This includes acetaminophen (Tylenol) and ibuprofen. It is the parent’s responsibility to notify the school immediately and submit a new form if there are changes in medication, dosage, time of administration, and/or the prescribing physician. Medication must be in the original container. Medication must be brought in by a parent or guardian and a signature is required at the time of drop-off. Two labeled containers from your pharmacist, one for school and one for home, can be requested. Medication cannot be administered at school except during the regular school day.
END OF THE YEAR MEDICATION RETURN
Unused medication can not be sent home with students. It must be picked up by parent at the end of the year and a signature is required at the time of pick-up. Medications not picked up will be discarded.
